Medical Records

  1. How do I obtain my medical records? That varies, if you are looking for your recent lab results you need to leave a message for the medical assistant. If you are looking for records for another doctor or you just want a copy of your records you must fill out a Records Release Out form. Once completed. you can either mail, fax, or bring the form in and give it to our front desk.
  2. How long does it tak to get my records? It can take up to 7-10 days to get your records provided you fill out the records release out form in it's entirety.
  3. Do I need to put my social security number on the form to get my records? No, it is not necessary unless you can not be identified by your first and last name as well as date of birth.
